Why Older Adults Should Visit the Mass General Brigham Community Care Van

Even if you have a regular doctor, the Mass General Brigham Community Care Van is here for you — bringing trusted, hospital-quality care right to your neighborhood.

As we get older, staying on top of our health becomes more important than ever. But we know that getting to appointments isn't always easy. Whether it's transportation, a busy schedule, or simply the hassle of waiting rooms, things can get in the way. The Community Care Van removes those barriers by coming to you — no appointment needed.

The van is staffed by a qualified medical team, including doctors and/or nurse practitioners, medical assistants, and community health workers — the same level of care you'd expect from Mass General Hospital, Brigham and Women's Hospital, or Salem Hospital.

Why It Matters Even If You Already Have a Doctor

Having a primary care doctor is wonderful — but visits are often short, infrequent, and focused on whatever is most pressing that day. The Community Care Van fills in the gaps:

  • Blood pressure can change quickly and silently. Getting it checked regularly — between doctor visits — can catch dangerous changes before they become a crisis. The team can even work with your provider to adjust your blood pressure medication on the spot if needed, and send you home with a blood pressure cuff for a closer eye on your readings.

  • Diabetes management is ongoing. A point-of-care glucose or A1C check and a conversation about nutrition and exercise can make a real difference in how you feel day to day.

  • Colorectal cancer screening is one of the most important things adults over 65 can stay current on. The van offers simple stool-based testing and can help you navigate a colonoscopy if needed — no separate referral process required.

What You Can Expect at No Extra Hassle

Walk-in visits are welcome — no appointment necessary. The team also connects you with resources if you have social needs like food, housing, or transportation support. A Cancer Patient Navigator is on board to help guide you through any follow-up care.

This isn't a replacement for your doctor — it's extra support, extra eyes on your health, and extra peace of mind. For older adults managing multiple conditions, that kind of consistent monitoring can make all the difference.
